In an effort to simplify things, we have streamlined all charting for infusion cases. It is now the same paperwork for any type of infusion visit! Effective November 16, the new paperwork must be completed for all infusion visits. It can be found in the Infusion Library in Zoho and here: Infusion Visit Progress Note. Please read our new policy on infusion visits and the new paperwork: 5.131 Infusion Visit and Documentation. *Important things to note:-In/out times need to be filled out at every visit.-Vital signs for visits without med administration will be charted on the Infusion Visit Progress Note.-Vital

Home Infusion: Teach and Train Visits Policy and Checklist

Hello team! Please take a look at our new policy for Teach and Train visits. It can be found here: 5.132 Infusion: Teach and Train and in the Infusion Library. The checklist that must be completed at all teach and train visits can be found in the Infusion Library and here: Patient Teaching Checklist. Note that it is not fillable because it needs to be printed, filled out, and left with the patient as a resource. Please take a photo of the checklist to upload to Zoho and leave the copy with the patient.

Home Infusion: New Paperwork for Routine Cases

What’s a routine infusion case? Routine visits can include any and all of the following: teach and train/re-teach, vascular access device (VAD) dressing change, lab work, or a medication administration/infusion that lasts less than 1.5 hours. Effective October 1, the new paperwork must be completed for all routine infusion visits. It can be found in the Infusion Library in Zoho and here: 5.131F1 Routine Visit Progress Note. Access and Use the Infusion Library

Hello all – One of the most frequently heard pieces of feedback we get is “communication needs to improve.” We plan to send out small chunks of info on a frequent basis, maybe weekly.  These should take no more than 30 seconds! Using the Library:  Most of our forms and information are just a tap away!  This video demonstrates how to access these resources in the mobile app. You can also visit cvaclinical.com from any browser.

VAT: Catheter Selection Tool

Hello all – We’ve recently updated the VAD Selection Protocol to include a Catheter Selection Tool. Choosing a catheter that occupies no more than 33% of the vein, with at least 2.75cm of vein purchase, will help us get better outcomes. Watch the 90 second video below to see how it works!  Kris

VAT: Consent for Midline

INS defines a Midline as “a catheter inserted into the upper arm via the basilic, cephalic, or brachial vein, with the internal tip located level at or near the level of the axilla and distal to the shoulder.”   Here at Chesapeake Mobile, consent is required for midlines.  Therefore, if the tip of your catheter, regardless of device, terminates at the axilla, consent is required. For charting purposes, ALL MST midlines are to be charted as Midlines, and both PowerGlide and AccuCath are to be charted as Extended Dwell Catheters.
