The checklist that must be completed at all teach and train visits can be found in the Infusion Library and here: Patient Teaching Checklist. Note that it is not fillable because it needs to be printed, filled out, and left with the patient as a resource. Please take a photo of the checklist to upload to Zoho and leave the copy with the patient.
