What Is Home Care

Home Care is care provided in the client’s residence by trained caregivers. Care is available in private homes, group homes, apartments, assisted living complexes or other senior housing environments. Care is supervised by our team of Registered Nurses, who visit at least every 90 days to assess progress and make adjustments as needed. Supportive private care is very flexible, and designed to meet most of the needs of daily life. It can be provided in conjunction with medical or “skilled” home care. Many clients seek supportive senior home care to enable them to continue to reside safely in their own home.

Customized Care Plans

Before any care is delivered, a detailed Care Plan is developed by a trained Registered Nurse, in cooperation with the patient’s physician, hospital discharge planner and family.
